The Controversial Birth of American Gynecology

As a woman, I am familiar with the gynecologist. In fact, thinking about it right now, I may need to create an appointment for one soon. However, I am not just a woman; I am a black woman, and in addition to being familiar with what the gynecologist is, I am also familiar with the dangers of the gynecologist. I know that if I were to become pregnant, I would be three times more likely to die by pregnancy-related causes compared with my white counterparts. This phenomenon is not new; in fact, it is a symptom of the racism within American Gynecology. The founding of this system is not pretty, or pure; it is ugly and distasteful, and during her lecture, historian Deirdre Cooper Owens explains it perfectly.

Cooper Owens is an associate professor of History and African studies at the University of Connecticut, and earlier this semester, she gave an insightful talk on how slavery and modern American gynecology are interconnected.

The controversial “father of gynecology” was J. Marion Sims, who experimented on enslaved women in Alabama. When talking about the racism in gynecology today and in the past, Sims mainly gets the blunt end of the stick. However, it was not just Sims; it was much bigger than him, Cooper Owens said.

Dr. Samual Cartwright was the first doctor for the Confederacy. Through his experiences with enslaved people, he believed that black people did not feel pain. Furthermore, he created a theory that if an enslaved person ran away or thought about running away, then they had a mental illness. Through the use of a spirometer (a medical tool still used today), he noted that black people have smaller lung capacity than white people. His findings were used to prove that there was a biological difference between races, which is not true.

This idea separated people and placed them in a hierarchy where white people were perceived as superior and black people inferior. The thought of this is damaging in itself, but back then, and sometimes now, they used this ideology as an excuse for the pain they caused African Americans.

Ephriam McDowell, for instance, removed a tumor from the ovaries of a white woman. From this, he then decided to “perfect” this surgery on five black women; four were enslaved, and one was freed. From this group, one person died, and other than that, there is no record of the women’s personal lives.

Dr. Francis M Prevost performed C-sections on enslaved people. These experiments did not take the pain of these women into account; due to the fact that he believed black people did not feel pain, but they did and still do. Now one would hope that a black woman’s relationship with C-sections has improved, but, from 1832 until two years ago, Louisiana was the state where a black woman’s body was used the most for a C-section. Today, that state is Mississippi.

John Peter Mettauer performed experiments on a white woman and a black woman. After the experiment, he claimed that the white woman was cured, but the black woman was not. As a result, he operated on the black woman eight times and claimed that if she did not have intercourse, she would have been cured. However, he failed to take into account that the woman was enslaved and had no control of her body autonomy. So how could she say no to both unwanted sexual encounters and to him?

Lastly, there is James Marion Sims, who is notorious for his contributions to American gynecology. However, such contributions were based on the bodies of enslaved women who had no choice. He used these experiments to advance his techniques and deepen his understanding of gynecology. In fact, it even went to the point where he built a hospital for the sole purpose of experimenting on enslaved women.

While the acts and experiments that these men conducted were atrocious, they raised a question for me, why black women? At that time, black people were viewed as an inferior race; they were not equal in physical components and intelligence compared to white people. Therefore, if they are genetically different, why experiment with black women to find cures for white women? When asking that question, the answer is obvious; they knew there was no difference, so they chose to ignore it. They chose to continuously bring harm to these women, and until recently, they were rewarded for it.

I learned a lot from this lecture, but if I had to choose only one thing that stuck with me, it would be that the victims of these heinous acts were only referred to as enslaved persons with no name and no story. The only story that was told was the point of view of those committing the acts.

I hope one day, the mortality rate of black women giving birth will decrease to the point that it is simply unheard of. Still, for society and our health system to reach that point, we must understand American gynecology’s true history.

COVID and Our Education

With mask mandates being overturned and numerous places going back to “normal,” COVID is becoming more of a subconscious thought. Now, this is not a true statement for the entire population, since there are people who are looking at the effects of the pandemic and the virus itself.

I attended a poster presentation for the “The Pandemic Divide” event hosted here at Duke by the Samuel Dubois Cook Center on Social Equity. To me, all the poster boards conveyed the theme of how COVID-19 had affected our lives in more ways than just our health. One connection that particularly caught my eye would be the one between American Education and COVID.

As a student who lived through COVID while attending high school, I can safely say that the pandemic has affected education. However, based on the posters I saw, it is important to know that education, too, has a strong and impactful impact on COVID-19.

The first evidence I saw was from Donald J. Alcendor, an associate professor of microbiology and immunology at Meharry Medical College in Nashville. His poster was about the hesitancy surrounding COVID-19 vaccines. One way he and his team figured out to lessen the hesitance from the public was to improve the public’s trust. To achieve this, Alcendor and his team sent trusted messengers into the community. One of the types of messengers they provided was scientists who studied COVID-19. These scientists were able to bring factual information about the disease, how it spreads, and the best course of action to act against it. Alcendor and his research team also brought in “vaccine ambassadors” to the community and a mobile unit to help give the community vaccines. He noted that this was accomplished with support from the Bloomberg Foundation’s Greenwood Initiative, which addresses Black health issues.

With this mobile unit, Alcendor and his team were able to reach people and help those who were otherwise unable to receive help for themselves because of their lack of transportation. They provided people from all backgrounds with help and valuable information.

Alcindor said he and his team planned pop-up events based on where the community they were trying to reach congregates. With the African American community, he planned pop-up events at churches and schools. Then for the Latino community, he planned pop-events where families tend to gather, and he held events in Latin0 neighborhoods. In addition, he made sure that the information was available in Spanish at all levels, from the flyers and the surveys, to the vaccinators themselves.

All of these amenities that he and his group provided were able to educate the community about COVID-19 and improve their trust in the scientists working on the disease. Alcendor and his team were able to impact COVID-19 through education, and by going to the event, it was evident to me that he was not the only one who accomplished this.

Colin Cannonier, an associate professor of economics at Belmont University in Nashville, asked and answered the question, “does education have an impact on COVID? Specifically, does it change health and wellbeing?” To answer this question, he researched how education about COVID can affect a person. He discovered that when a person is more educated about COVID, how it is spread, and its symptoms, they are more likely to keep the pandemic in check through their behavior. He came to this conclusion because he realized that when higher educated people know more about COVID, they exhibit behaviors to remain healthy, meaning that they would follow the health protocols given by the health officials.

While this may seem like common sense that the more educated a person is, the more they make smart choices pertaining to COVID, this shows how important education is and how deadly ignorance is. Cannonier’s research gave tangible evidence to show that education is a weapon against diseases. Unfortunately, it is evident that some officials did not believe in educating the public about the virus or the virus itself, and that proved to be extremely deadly.

To fully capture the relationship between COVID and education, one must also talk about how COVID-19 affected education.

Stacey Akines, a history graduate student at Carnegie Mellon University, studied how education was changed by the pandemic.

First, she realized that COVID schooling crossed over with homeschooling. Then she uncovered that more Black people started to research and teach their children about Black history. This desire to teach youth more about their history caused an increase in the number of Black homeschoolers. In fact, the number of Black homeschoolers doubled during the fall of 2020. While to some, this change to homeschooling may have a negative impact on one’s life, it actually gives the student more opportunities to learn things.

It is no secret that there are many books being banned here in the U.S., and there are many state curriculums that are changing to erase much of Black history. Homeschooling a child gives the parent an opportunity to ensure that the education they receive is true to and tells their history

Unlike me, where during high school, education felt lackluster and limited because of COVID, some parents saw an opportunity to better their child’s education.

I hope that it is clear that the relationship between COVID and education is a complex one. Both can greatly impact each other, whether it’s for the better or for the worse. COVID thrives when we are uneducated, and it very nearly destroyed education too, but for the efforts of some dedicated educators.

Banned Books Then and Now

With the never-ending news of schools banning books, one has to ask about the history and effectiveness of banned books. Fortunately, the Duke Forever Learning Institute hosted a seminar on the history of banned books in late September and provided examples that showed the ineffectiveness of banned books throughout time.

There was one particular story, however, that caught my eye, and that was the retelling of the history behind a banned book written by a man named Gottschalk. Clare Woods, an associate professor of Classical Studies at Duke, delineated the narrative of Gottschalk’s life from birth to prisoner.

Gottschalk was born to an aristocratic family around 803 AD and started his academic career when he moved into Fulda, a monastery, to become a monk. However, in 829 AD, Gottschalk left Fulda for reasons based either on the loss of his older brother or the enlightenment he received from the monastery he attended while studying abroad.

Upon leaving Fulda, Gottschalk studied at the monasteries of Corbie and then moved to northern Italy after he received his orientation as a priest. Gottschalk was unusual because monks tended not to move around that much, and monks also tended not to speak about their studies on heretical doctrine. Unfortunately for Gottschalk, he ignored the criticisms from other monks and instead grew confident in his studies to the point where he wrote a book about them.

While receiving hate for his ideas from multiple people, one man from Gottschalk’s past was truly adamant about having Gottschalk punished. This man was named Rabanus (Hrabanus), and he was accused by Gottschalk of coercing Gottschalk’s earlier career as a monk. From that, and with Gottschalk leaving the Fulda, Rabanus developed a personal vendetta against Gottschalk.

This is an excerpt from Gottschalk’s confession, which was provided by Dr. Clare Woods.

Gottschalk, the confident man he was, decided to defend himself at a Synod in Mainz, but it proved unsuccessful because he was condemned by the Synod and banned from the Kingdom of Louis. From there on, things did not improve for Gottschalk because, in the following year, he was brought to a second Synod that was attended by King Charles, who was the brother of King Louis the German. By the end of this Synod, Gottschalk was stripped of his priesthood, sentenced to silence, and imprisoned.

However, even with the punishments Gottschalk personally received and his books being burned, his books were still preserved and influential on others.

This influence seen with Gottschalk’s books after being banned was not an anomaly because that was seen with all banned books throughout history.

Another example was given by Lauren Ginsberg, an associate professor of classical studies, who presented Permussius Cordis, an author who lived under the Roman emperor Tiberius in the 1st century AD.

His story ended in tragedy since his books were included in a mass book burning and he was sentenced to death. However, his book still was able to influence the Roman population due to his daughter Marsha’s vigilance in keeping her father’s book alive.

While I only provided two examples, this pattern of books withstanding their ban is throughout time, and it is being repeated in the present. Across this country, book banning is back in style. In 2018, there were only 347 books that schools formally challenged, but in 2021 it became 1,500.

While this does put light on our current education system in this country, this is simply a gesture. It does make us think about what children will be learning in schools, but because the books are banned in schools does not mean the information within those books will not reach their target demographic. As seen in history, knowledge always finds a way of spreading, but of course, that is dependent on those who want to expand that knowledge.

At the end of this article, I hope you are reassured that seeing a banned book does not mean it is forever silenced. Instead, I hope that by reading this, you understand that you have all the power to ensure that the words within a book can withstand anything, including time.

The Need for Title IX in STEM

The Panel:

In recognition of the 50th anniversary of Title IX, which was intended to make sex discrimination in education illegal, a panel of Duke women met on Thursday, September 29 to talk about whether Title IX could change STEM, (Science Technology, Engineering and Math). Unfortunately, the answer was not simple.

But just through the sharing of the statistics relevant to this problem, the stories, and their solutions, one could start to understand the depth of this problem. One takeaway was that all women in STEM, whether they be student, professor, or director, have faced gender discrimination.

Down to the Statistics:

Dr. Sherryl Broverman, a Duke professor of the practice in biology and global health, gave the audience an overview. Of all of Duke’s regular ranked, tenured-track faculty, only 30% are women. In contrast, women make up 60% of the non-tenure track faculty. Dr. Broverman said men are promoted in Duke at a higher frequency. This is especially seen with the associate professor title because, on average, men are associate professors for 4 to 5 years; whereas women are associate professors for up to 9 years.

To give an example, senior Nasya Bernard-Lucien, a student panelist who studied Biomedical Engineering and then Neuroscience informed me that she has had a total of two women professors in her entire STEM career. This is a common pattern here at Duke because taking a STEM class that has a woman professor is as rare as finding a non-stressed Duke student.

The Beginning of a Girl’s Career in STEM

This disproportionate demographic of women professors in STEM is not a new occurrence with Duke or the rest of the world because the disproportion of women in STEM can be seen as early as middle school. Two of the student panelists noted that during their middle school career, they were not chosen to join an honors STEM program and had to push their school’s administration when they asked to take more advanced STEM classes.

Dr. Kisha Daniels, an associate professor of the practice in education said on a faculty panel that one of her daughters was asked by her male peers, “what are you doing here?” when she attended her middle school’s honors math class. Gender discrimination in STEM begins in early childhood, and it extends its reach as long as women continue to be in a STEM field, and that is particularly evident here at Duke.

The last panel of the Title IX @ 50 event was the student panel which consisted of undergraduate and graduate students. Even though they were all from different backgrounds, all acknowledged the gender disparity within STEM classes.

Student Bentley Choi said she was introduced to this experience of gender discrimination when she first arrived at Duke from South Korea. She noted how she was uncomfortable and how it was hard to ask for help while being one of the few women in her physics class. One would have hoped that Duke would provide a more welcoming environment to her, but that is not the case, and it is also not an isolated incident. Across the panel, all of the women have experienced discomfort in their STEM classes due to being one of the few girls in there.

The Future of Title IX

How can Title IX change these issues? Right now, Title IX and STEM are not as connected as they need to be; in fact, Title IX, in the past, has been used to attack programs created to remedy the gender disparity in STEM. So, before Title IX can change STEM, it needs to change itself.

Title IX needs to address that this problem is a systemic issue and not a standalone occurrence. However, for this change to happen, Dr. Whitney McCoy, a research scientist in Child and Family Policy, said it perfectly, “we need people of all backgrounds to voice the same opinion to create policy change.”

So, talk to your peers about this issue because the more people who understand this situation, the chances of creating a change increases. The last thing that needs to occur is that 50 years in the future, there will be similar panels like this one that talk about this very issue, and there are no panels that talk about how we, in the present, fixed it.

Meet New Blogger Jakaiyah, an infectious disease enthusiast

Hello, my name is Jakaiyah Franklin, and I am a sophomore here at Duke University. In terms of my major, I am undecided, but I do know my passion lies in biology, science communication, and environmental science.

Outside of classes, I am the treasurer for the Duke Chapter of the NAACP and LLC leader for the Stem Pathways for Inclusion, Readiness, and Excellence (SPIRE) program. Last year I was the stage manager for two Hoof n Horn productions.

This is the Rocky Horror Picture Show company after finishing our last show.

This year, I will start a research position along with this research blogging position.

In a more personal sense, I am the youngest of three and a proud aunt. Right now, I say I am from Texas, even though I have lived in Georgia, South Carolina, Germany, and presently North Carolina. If someone ever asked me, I would say that Germany holds my most memorable memories; however, I have grown into a better version of myself in each place I have lived. Other than school, I like to read and watch House of the Dragon and the earlier seasons of The Game of Thrones. I prefer to study outside or in a place where natural light is abundant. I also love learning new things pertaining to science, specifically infectious diseases.

My view of Muhuru Bay, Kenya, where I spent the summer after my first year at Duke. That’s Lake Victoria in the distance.

I find diseases fascinating, and I believe they are our natural predators. I want to be able to not only understand them, but also, I want to help prevent them. If one were to have a favorite type of disease to study, mine would be zoonotic diseases. They are interesting because the act of a virus being able to jump from a host like a rat to a human is captivating to me.

After graduating from Duke, I want to earn a master’s in public health or a Ph.D. in epidemiology, virology, or infectious disease to feed my curiosity about diseases. However, before I can even decide what Ph.D. or master’s I want to earn, my current goal is to decide on my major.

I do like to think ahead, so, for my very distant career, I know I want to be able to see infectious diseases in both the lab and in the places where they are infecting populations. I want my research to be digestible for the general population because, as seen with both COVID and Monkeypox, science can be easily misinterpreted if not delivered appropriately. I want to prevent this occurrence from happening to me by learning more about science communication and actively improving my communication skills.

I hope this blogging position will expose me to infectious disease research or general public health research. With this new understanding of the research, I hope this position will also educate me on how to inform others so that they can enjoy and understand the science.
